Distribution: To all customers and users of the system
During the “check-before-use” of an individual fall arrester of the RS-S 05 height access protection system as part of a drill, it was discovered that the shaft of the cardan joint was not secured (secured by expanding / compressing the ends of the shaft).
Risk: In the worst case, this can lead to the karabiner hook becoming detached from the fall arrester and could potentially cause a serious accident!
Probability: According to our research, this is a one-off incident. There is no problem with the materials or the process. More than 40,000 such devices have been manufactured to date without a single occurrence of such a fault!
Action: As a company in which safety is an extremely high priority, we would like to notify our customers of this extremely unlikely risk. For the sake of safety, please check that the shafts are firmly seated before continuing to use the fall arrester:
- The shaft must not move in the longitudinal direction
- The rivet head of the shafts must be 8.4 – 8.7 mm in size

If you have any doubts about the equipment, stop using it immediately and have it checked by an expert or the manufacturer!
Take defective equipment out of service and return it to BORNACK for reworking!
As a result of this incident, BORNACK will be systematically reviewing its manufacturing procedures in order to implement possible optimisations in all areas. BORNACK is therefore once again exceeding the high safety requirements of EC directive 89/686 for class III products. This will result in even more safety for our customers.
